Reservations limited. Payment MUST accompany this registration form in order to have your golf spot confirmed. The Tournament committee will pair reservations with less than four players. Registration is good for the tournament date requested and confirmed only. The fee is non-refundable. The tournament will be played rain or shine, but may be cancelled in extreme weather conditions. Checks should be made payable to; CSMB and sent to 26 Broad St, Milford, CT 06460.
